In order for your child in grades 5-8 to participate in sports, they must have the following completed/turned in:

• Sports Physical 
• Medical Consent and Indemnification Agreement Form (find below)
• Copy of medical insurance card
• Athletic fees: $20 for cheerleading; $30 for cross country; $40 for volleyball; $50 for basketball. Please make checks payable to STM Athletics.
